Before completing this application form,please carefully read the Program Guidelines, Frequently Asked Questions, First Nations NSW Confirmation & Connection and Aboriginal Arts and Culture Protocols available on the website.

Prepare your application with this checklist
All applicants must submit the following materials. All materials will form part of the assessment:
  • A completed online application form
  • Samples of work (eg. attach images, short video files, PDF, 25MB max)
  • A detailed artistic and/or service plan of activities in 2026 (template below)
  • Organisational structure (if confirmed)
  • Audited financial report or equivalent for 2024*
  • Projected budget for 2026, including a breakdown of funding sources (optional budget template provided).

*If you do not have your accounts audited, you can upload a Balance Sheet and Profit and Loss statement that has been certified or approved at your most recent AGM or Board meeting.

Create NSW may also use previously submitted acquittal and forward plan data and information to inform the decision making of this funding round.

Create NSW, part of the Department of Creative Industries, Tourism, Hospitality and Sport (The Department) is collecting your personal information for the following purposes:

  • To assess your application against the eligibility criteria for this grant round of the Arts and Cultural Funding Program;
  • If your application is deemed eligible, for Create NSW to subsequently administer the Program;
  • For Create NSW to publish your personal information in a way it deems necessary in order to publicise your connection to the Program; and
  • For Create NSW to monitor and evaluate the Program.

Create NSW will not use your personal information for any other purposes. Providing your personal information to Create NSW is voluntary, however Create NSW may not be able to assess your suitability for the Program if you choose not to provide it.

Create NSW will not disclose your personal information to anybody else unless you have given your consent, or Create NSW is required or authorised to by law, such as by court order or under the Government Information Public Access Act 2009 (NSW) (GIPA Act).

You have the right to access your personal information and/or correct your personal information held by Create NSW without excessive delay or expense.
